India’s Immunisation Triumph: Zero-Dose Rate Drops to 0.06% in 2024

A Remarkable Turnaround in Child Vaccination
India has achieved a major milestone in its public health journey: the proportion of zero-dose children, those who receive no doses of the vital DTP vaccine, fell dramatically from 0.11% in 2023 to a historic 0.06% in 2024. This success, celebrated by the UN Inter-agency Group for Child Mortality Estimation, marks India’s rapid transformation into a global exemplar in child immunisation efforts. This drop signifies millions more children gaining access to foundational vaccines, a public health leap seldom witnessed in countries of comparable population size.

Health Ministry Lauds National Programme and Digital Push
India’s Universal Immunization Programme (UIP) stands at the core of this achievement, delivering free vaccinations to an astonishing 2.9 crore pregnant women and 2.6 crore infants every year. The Ministry credits a proactive, inclusive approach, door-to-door campaigns, digital record-keeping via the U-WIN platform, and mass awareness messaging, for bridging gaps in hard-to-reach communities. Frontline workers, ASHAs and ANMs, conducted over 1.3 crore vaccination sessions nationwide, ensuring the tiniest wards aren’t overlooked.

Mission Indradhanush: Reaching the Unreached
Launched in 2014 and intensified in 2017, Mission Indradhanush has played a pivotal role. Coordinated UIP drives led to the inoculation of 5.46 crore previously unvaccinated children and 1.32 crore pregnant women by 2023-24. Its intensified phases have particularly focused on mobile, urban poor, slum, tribal, and marginalized groups, communities often missed by traditional healthcare access. In 2024, with additional rollout of the Zero Dose Implementation Plan across 143 high-priority districts spanning 11 states, the government doubled down on eliminating access disparities.

Umbrella of Innovation: Digital Tools and Cold Storage
India’s digital ecosystem, eVIN and U-WIN, has underpinned this public health leap. eVIN ensures real-time monitoring of cold-chain vaccine storage across 30,000 logistic nodes, while U-WIN digitally tracks immunisation records, schedules appointments, and issues QR-based certificates. Launched in 2024, U-WIN now registers over 23.5 million pregnant women and 67.7 million children and has overseen nearly 375 million doses, an impressive digital leap in ensuring no child slips through the cracks.

Equity and Last-Mile Focus
While urban centres typically show stronger vaccination coverage, India has confronted the “last-mile” challenge head-on. In Arunachal Pradesh’s remote districts, health workers have even used elephants to access isolated families. In Maharashtra’s slums, grassroots partnerships with community and transgender leaders have helped overcome stigma and misinformation. Tribal belts and conflict-affected zones are receiving continuing attention through culturally sensitive campaigns and community engagement.

Global Recognition and Contextual Insight
In global terms, India historically recorded the second-highest zero-dose child count, 1.6 million in 2023, but as a percentage of its 1.4 billion population, this translated to only 0.11%. The recent drop to 0.06% highlights India’s impressive coverage, significantly above many countries, which the government maintains is a critical context when comparing absolute numbers.

Vaccine Portfolio Expansion and Wider Impact
India has consistently enhanced its vaccine offerings. Six new vaccines, including Rotavirus, Pneumococcal conjugate, injectable polio, JE, PCV, and MR, have been integrated into UIP since 2014. The first-dose MR coverage already exceeds 93.7%, resulting in an impressive 73% drop in measles cases in 2024. This broad vaccine slate helps tackle childhood pneumonia, diarrhoea, meningitis, and more, amplifying impact beyond DTP statistics.

Public Health Milestones and Economic Value
Through consistent investment in immunisation, under-five mortality has plummeted from 126 per 1,000 live births in the early 1990s to just 32 per 1,000 in 2023. India’s current full immunisation coverage stands at an impressive 93%, placing it alongside high-income nations. Economically, every dollar invested in vaccines returns up to $44 in savings, through reduced illness, hospital burden, and improved lifelong productivity.

The Path Ahead: Sustaining Success
Despite these gains, sustaining momentum is critical. The Health Ministry pledges ongoing door-to-door drives, digital tracking, targeted campaigns in underserved regions, and mass awareness via media and community leadership for 2025 onward. There’s also a push to leverage CoP-D (Community of Practice for Demand), bringing together local influencers, non-profits, and health workers to tailor community-level interventions.

Conclusion: A Triumph Rooted in Strategy and Empathy
India’s steep drop in zero-dose children is a testament to determined policy, digital innovation, and culturally grounded community outreach. The synergy of Mission Indradhanush, UIP, U-WIN, and targeted implementing strategies with local community involvement charts a compelling roadmap for other nations. As India eyes further reduction in missed children and fuller immunisation coverage, its story becomes a powerful narrative of turning public health vision into reality, and delivering equitable health to every child.
